Hello everyone. First of all, I want to thank you for taking time to answer folks like me here and giving us your opinion.

I turned 20 5 days ago. My symptoms started in October 2023, when I noticed fasciculations in my feet and a pretty weird symptom of scalp spasms (feels like a vein is popping out of the top of the head, it could also move like a worm). I didn`t give it any thought, took some vitamins and it seemed like they dissapeared. I also did a blood test and had ALT and AST levels elevated.

In the beginning of November 2023 after mild cold I noticed twitching in my right leg. I thought this was because of the medication or something else, so I never gave it any thought. Then I started having forehead spasms that felt like there was something inside (like sinusitis. I even went to the ENT doctor, but they found nothing and referred my to the neurologist. 2 days after that i started having twitches and spasms all over my body, no cramps. i went to see a neurologist, because I was scared of potential ALS. it is important to notice that I never had major fasciculations like some people with BFS have, for example in a calf. The majority of it were spasms that in 90% of cases weren`t painful. (They were everywhere: legs, arms, back, belly, and even scalp and tongue)

She did an exam and didn`t find anything abnormal, after that she reffered me to EMG and it was all clean, they didn`t even find fasciculations (which I think is weird).
During November 2023 I`ve had on and off fatigued feelings of different limbs, but nothing too serious. But the last week I stood up after a lecture at university and my leg had the stiffness/weakness I`ve never experienced before. When I was walking to the bus, I had to walk with that leg like I was walking over something (i don`t know how else to describe it). Also my left thumb started feeling stiff and tired. It Both leg and thumb still feel the same.

I also started having muscle pain over the whole body (I guess due to spasms), sometimes it feels like I have a flu because of a similiar muscle pain.

I was at neurologist`s office, told her about the symptoms. She did an exam, some streignth testing, and didn`t find anything abnormal. She said it was all psychological and didn`t even bother to refer me to MRI or anything else. I guess her mantra is "wait and see" and she`s just waiting to see if what I have will progress. I also did a blood test again and my ALS and AST levels were normal.

Today I noticed something scary in my biceps. When I flex them and then relax and extend, they start to shake. My spasms also started hurting more and sometimes I feel like my calf is on the verge of cramping. It would be very kind of you if you could answer some questions, not just for my own "diagnosis", but also because i`m curious about the ALS and non-ALS symptoms.

1. Is this bicep shake anything pathological?
2. I`ve read a couple of reddit posts of people claiming that if you have twitches above the eyebrows, then its not ALS, because you don`t have motor neurons on your forehead and top of the head, but they never gave any source of that information. Considering my own symptoms, do you or your loved ones have/had anything similiar (meaning scalp twitches and spasms)? And is this claim true?
3. Could my case be UMN onset? I`ve read stories of people with UMN onset, but they presented with cramp that would lock their muscle and they weren`t able to move it. I guess I have something similiar in the leg, but not that severe.

To address your questions directly:

1) That's a very broad question, and one better answered by your doctor, who can observe what you mean by shaking. You've been cleared of ALS so you will have to discuss with them if it means anything concerning.

2) You will read all sorts of things about twitching online, where, how often, etc, etc. You have been cleared of ALS by your doctor, so you will have to look at things other than ALS for causes of twitching.

3) No, this could not be UMN onset because you are not describing UMN symptoms in any way.
