Hello everyone! I am a 29 yo man. Since the beginning of November/24, i have been feeling weak in my right leg. At the same time, I started having fasciculations. Initially, they were only in my leg, but over time they appeared in other areas, but less frequently. Over the past few months, the fasciculations have diminished considerably. What worries me is the persistent feeling of weakness in my right leg. I can still lift the same weights at the gym on both sides, but the right side always seems to get tired more. I run3 times a week (about 35 km per week).
I’m still scared by this feeling of progressive weakness in my right leg. Could this be progressive weak? Please someone helps ;/
I had an EMG done on four limbs after a month a half the symptoms started. No changes were found.

It explains why twitching absent clinical weakness does not mean ALS. In addition to a clean EMG, you are good to go.

Couldn’t the EMG have been too early?
 
No, not in the scenario you describe. I would ask about an X-ray of your leg and a physiotherapy evaluation.

EMGs cannot be done too early. 70% of a nerves fibers die before you feel weakness. An EMG will detect that process long before you can feel it.
Here is a simplified summary:

1. ALS causes nerves to die.
2. EMG detects effects of nerves dying.
3. For ALS, muscles get weak and atrophy when they lose their nerves (no pun intended).
4. Therefore a weak or atrophied muscle due to ALS will absolutely, positively show up on EMG. Clean EMG = No ALS and a clean EMG plus a normal clinical exam = no Motor Neuron Disease at all
 
Hello everyone! I am a 29-year-old man.

Since the beginning of December 2024, I have been experiencing weakness in my right leg. At the same time, I started having some fasciculations. Initially, they were only in my leg, but over time they appeared in other areas, although less frequently. Over the course of these months, the fasciculations have decreased considerably, with occasional hot spots (the last one was in my right shoulder, lasting almost 10 days, almost 24/7).

This weakness in my right leg has been increasing. It tires more easily and remains fatigued for longer periods of time. I can still lift the same weights at the gym on both sides. I run 3 times a week (about 35 km per week). But this sensation worries me. 1 month and a half after the onset of symptoms, I had a neurological consultation (no changes in the neurological examination) and an EMG on four limbs, without changes. Does this feeling of worsening weakness suggest a possible progression to clinical weakness?

I understand. Currently I don't have any fasciculation point that is 24/7. They are like a quick twitch in random areas of the body. They are not constant. 10 days ago I started to feel, at times, pre-cramp sensations in my right leg. Since the beginning of the episode I have had the sensation of having more saliva in my mouth, needing to constantly swallow.

My fear is the perception of constant weakness in just one leg
 
Do not be so quick to connect saliva and cramps. Anxiety about one can easily lead to the other, and go back and forth.
