Hi all. Thank you for your time. I think my symptoms may sound like ALS and appreciate any feedback.

38, female. 3 weeks ago I awoke with sudden drop foot. Total loss of dorsiflexion in right foot. No pain, no injury, no excess use, no buildup, just... drop foot. There is a weird sensation on the front of my shin where the muscle isn't working. Not fully numb or tingly, just sort of different.

MRI brain, spine, and lumbar all normal. Some cervical stenosis but nothing to account for drop foot. Referred to neuro for EMG, which is booked 4 months out.

Have had nerve pain before, I call it "marathon legs", where my legs would feel like I had run a marathon that day. It was never a big deal. Have complained of loss of hand strength and arthritis pain in hands for years, thought it was carpal for a long time, until Ortho said nope. I assumed general neuropathy.

My right butt was starting to hurt, so went back to the ER. They told me maybe piriformis syndrome and to get to neuro. Was put on a Prednisolone pack, which has had no effect other than my butt being less sore.

Right buttock, hip, and thigh still don't feel great, but could just be sore from compensating these last few weeks.

Noticed yesterday my legs were feeling extra twitchy, superficial small twitches on both legs, mostly all over my thighs, after walking. I didn't think much of it other than I noticed.

I didn't know the connection between twitches and drop foot and ALS until tonight. Now I notice more twitching, in different places on my body. Anxiety induced? Idk. Radiologist spine report mentioned patient movement, which I thought was bs as I was so still. Now I'm wondering if I had twitches on the table that I just didn't notice.

I know I need an EMG, it's just a long wait. Meanwhile I'm getting really scared that this might be ALS. I guess I'm just reaching for anything or anyone to refute that fear. I keep hoping for pain to return so I can point to some other sciatica type thing, but it's not really pain, just discomfort in my thigh/hip on my drop foot leg.

The most common thing we see here with your presentation is peroneal nerve compression which wouldn’t show on mri. Twitches as you say can be a manifestation of anxiety or a result of nerve irritation or just plain benign.

I am sorry you can’t get an emg soon. Most everyone has to wait unfortunately
 
Thank you. Hoping it is just nerve compression, though I'm not sure what could cause it like this. Will hope for the best and continue to seek evaluation.
