Hello, my name is Sam and let me start by saying like many people here, I have anxiety which lately due to my symptoms has been off the charts. About two years ago my symptoms started with a twitching eyelid for two weeks which turned into right sided perceived weakness (arm and leg) and heaviness for two weeks longer before I went to the ER. I was told I have migraines with aura as well as c5-c6 stenosis and sent on my way with PT. Due to this I pushed for further testing with a neuro who got me a EMG and here are the results .

Summary of NCS:
Motor NCS:
Right fibular CMAP demonstrates normal distal latency, low amplitude, and normal conduction velocity.

Late responses:
Right median F-wave: WNL
Right ulnar F wave: WNL
Right tibial F wave: WNL
Right fibular F-wave: prolonged
left fibular F-wave: WNL

Right biceps femoris,tibialis anterior, tibialis posterior, and medial gastrocnemius muscles showed normal insertional activity and result of normal spontaneous activity at rest. The motor unit potential morphology was abnormal with large amplitude and polyphasic potentials.

Impression: The electrodiagnostic examination of the right upper and right lower extremities showed the following: Presence of the right L5 and S1 motor radiculopathy, chronic in degree electrically There is no electrodiagnostic evidence suggestive of large fiber polyneuropathy. Clinical correlation is advised

This led me down a rabbit hole and there I found ALS, something I never looked into before and ever since has taken over my life. Once I got COVID 9/23 for the second time all my symptoms and quality of life is gone. I used to workout 5 days and week but then noticed around December when I tried tk start working out again, am out of breath immediately (think one flight of stairs) and am unable to lift as much on my right as my left (I am right dominate). So I requested another EMG and was given a Somatosensory Evoked Potential test with these results in 11/23:
Somatosensory evoked potential studies in the bilateral tibial nerves show normal latencies at the lumbar and cortical P37 on the left side. The lumbar potential on the right side is mildly prolonged. P37 on the right side is normal. Interpeak latencies are normal bilaterally
Impression: Mildly abnormal somatosensory evoked response of the right lower extremity due to mildly prolonged lumbar potential suggestive of a peripheral process. No evidence of central pathology was seen bilaterally.

After these results I felt like no one would listen to me. But the last two weeks everything has gone downhill drastically. It started with my tongue feeling like I was having an allergic reaction, it felt tingly and strange and when looking at it I noticed it was twitching as well. This lasted days until it was accompanied by muscle cramps that last 5-10 seconds all over my body and often two or three locations at the same time (fingers, feet, calves, back, arms, abdomen, chest, etc). I don’t usually see them but feel them intensely under the skin. When I smile my mouth quivers and my tongue feels tired after talking short periods. Now I am starting to have tremors as I keep shaking my hands and feet and just feel an all over vibrating feeling that won’t go away. My arms feel fatigue with little movement and lifting things I used to or even using my phone brings on fatigue in my arms and I need to take a break. It feels like my world is crumbling and no one is listening. I have a doctor appt in a few weeks and she has ordered some basic labs but I fear this won’t do much to help my cause. I guess I was hoping for any suggestions that can be provided or further testing I should ask for. I feel hopeless and lost. Thank you.
 
Discussing further testing would be between you and your doc, but I can assure you that you're in the wrong place here. You are not describing ALS or anything like it.

Feeling like your world is crumbling, in your words, suggests that your sleep, stress, bodywork, and diet are not where they should be for good health. And it's a Catch-22 when you start worrying about your health generally. So those are the things I would work on, including counseling for your ALS concerns. The EMG and SSEP demonstrate nothing seriously wrong, but the EMG suggests that you might benefit from PT focusing on your lower spine.
