My symptoms are:

Whole-body twitches that comes and goes (not that worrying about this unless it concentrates to where weakness is)

Left side is a lot more tremor-y under load (such as when I lift a 5kg dumbbell from relaxed arm to in front of me)

Unilateral self-tested Hoffman sign on right (doctor did not test that in my visit and said she don't care about that, Hoffman is not really that important in her opinion)

Left pinky toe a lot more tremor-y than right and has near clinical weakness

strong whole body tremor occasionally at night when right before sleep or right after awake, once per month, starting from the left side. (this does not concern me that much since whatever comes and goes is not usually in ALS visual field, but left side heavy tremor, well)

Doctor cleared me after a basic clinical exam, saying that major muscles does not have clinical weakness, which I already know, and saying that my risk level is low. But-what about tremor? I heard in anecdotal account that someone get diiagnosed because arm holding in front tremor, albeit that is not under load

Did not dare to do EMG, the doctor said that EMG is not that important for me too though
 
Other than possibly suggesting some torso exercises to build strength, no reason to think about further care that I can see, and certainly don't see your reason for concern re ALS.

Anecdotal accounts are stories, and should be treated as such.

Now I think I have perceived speech difficulty

I can still pronounce the word "Shell" clearly and other did not hear problems in that sound. However, It felt off and the word Shell seems to take more effort to pronounce. And if I spell it very fast I may mix up and fail to pronounce it. I think I have more "perceived trouble" in "Shell" more than say "Banana"

I know that ALS is mainly about failure, not feeling, and I am not even sure that if this is yet another hyperfixation just like the previous ones. But I read that ALS starts as "minor speech difficulties" so I don't know if this is concerning

I went to neuro for perceived hand weakness but clinical revealed nothing one month ago. The neuro also looked at my tongue and don't think my speech and tongue has anything concerning. But that's one month ago, I don't know now. Now I felt that my speech is a little more off at least due to hyperfixation

Does this sound like Bulbar ALS?
 
No, not to the extent that shell is a tricky word to start with and others are not hearing problems. In speech problems related to ALS, others hear it, too.
